All HRVHS Local Scholarship applications require two to three documents. Students will submit one copy of each of the following documents. Your transcript and Activities Chart will be shared with scholarship committees. Your FAFSA/ORSAA is confidential — only your Expected Family Contribution (EFC) will be shared with scholarship committees.
Students will submit the following:
- HRVHS Transcript (include fall semester)
- Oregon CIS Activities Chart
- FAFSA SAR or ORSAA (optional see FAFSA section below)
NOTE ON FORMAT: All of your common documents must be uploaded as a pdf. Do not:
- Take a photo of a printed page and upload it
- Upload files in the .pages format or other apple / mac document types as windows users can’t open these documents.
Scholarship committees may consider your application incomplete or unprofessional if you do not upload your common documents as a pdf. If you need help with saving your documents as a pdf, visit a writing lab or contact your school’s tech support.

Activities Chart
All students will need to submit one copy of their Activities Chart as a pdf. Students are limited to ONE PAGE, front and back for their activities chart.
FAFSA SAR or ORSAA
The FAFSA or ORSAA is an optional common form that students may choose to submit along with their Transcript and Activities Chart. The FAFSA / ORSAA is required for some (but not all) of the local scholarships, particularly scholarships for students with financial need.
If a student decidees to submit their FAFSA / ORSAA, they will need to submit EITHER their FAFSA Student Aid Report (SAR) OR the first and last page of their ORSAA application. Do NOT submit the entire FAFSA / ORSAA document as it contains sensitive information. Instructions on obtaining these forms and saving the correct pages can be found below.
Your FAFSA / ORSAA must list your full name and your expected family contribution to be accepted.
Notes:
- Students who do not upload their FAFSA / ORSAA information will not be able to see or apply for scholarships that require the FAFSA
- Students who have financial need are strongly encouraged to submit their FAFSA / ORSAA. Many scholarships consider financial need, even if they do not require the FAFSA as part of their application.
Your FAFSA / ORSAA is confidential and will not be shared. The only information shared from your FAFSA or ORSAA is your Expected Family Contribution (EFC) or Student Aid Index (SAI). The FAFSA / ORSAA document is deleted after your EFC / SAI is verified by HRVHS staff.
Your documentation status is confidential. If you have any concerns, please reach out to Ms. Ownby or Ms. Bauman.
